After submitting your appointment request
We will attempt to contact you the same business day to schedule your appointment.
Intake Forms
We will send you an email with a link to our patient portal. When you first go to the portal you will be given several consent documents to read and sign. You will also have other forms to complete including a credit card authorization form and a patient health history form. All of these must be completed before your appointment. It is preferable to complete them a day or two before your appointment so that so they can be reviewed before your visit.
Upload Reports from Previous Care Providers
It is important to upload any documentation related to your issue. This includes prior visits, tests/image results and reports. As with the intake forms, please upload before your appointment day. Reports can either be uploaded via the patient portal or faxed to (804) 408-3335. Imaging results on CD can be brought to the appointment.
Appointment FAQs
What should I wear?
It is best to have loose clothing; bring a pair of gym shorts and/or sports top to allow assessment of areas of injury. If you are traveling to visit from work, there is a restroom in which to change before your appointment.
What if I don't have the records from my previous visits?
You will need to contact your provider and ask for the records. You can then upload reports and results to the patient portal.
How can I get the records to you?
If you have time, you can drop them off at the office so the doctor can review them before the appointment. You can also bring them to the appointment and the physician can review them during your appointment, however it will limit the time available for evaluation and treatment.
I was given a disk with the imaging results. How do I get them to you?

How and when do I pay for my appointment?
One of the forms will be for your credit card information. We will bill the card on the day of the appointment automatically unless you request to pay at the time of service. We can take a credit card or a check at the office.
Can I submit my bill to my insurance?
We elect not to accept or work with health insurance companies or third-party payers. Patients pay for services at the time of the appointment. However, patients do have the option of requesting a detailed invoice to submit to their insurance company for out-of-network reimbursement or reimbursement through an FSA or HSA plan. The first step is to contact your insurance provider to see if you have out-of-network benefits.
I have Medicare. Can I submit a claim to Medicare?
No you cannot. We have opted out of medicare. If you are enrolled in Medicare, we are required to have you sign a Medicare Opt Out Agreement as one of your intake forms. It is your acknowledgement that you understand we do not participate in Medicare and that you will not submit any claims to Medicare for reimbursement.
What if I have to cancel or reschedule my appointment?
If you need to make any changes to your appointment, let us know as soon as possible – ideally within 48 hours of the scheduled appointment to avoid a late cancel fee.
